Patient Forms
At Valley Digestive Health Center, your time is as valuable to us as it is to you. That's why we provide online access to a variety of forms that patients may complete before their visit. The following documents will help us expedite the check-in process for your appointment. Please take a moment to download and print the appropriate form(s).
If you would like to schedule your colonoscopy over the phone, please follow the instructions below:
1. Download and fill out the Patient Information Sheet, Pre-Colonoscopy History and Privacy Practices Acknowledgement.
2. Please include a copy of your Insurance Card (if that is not possible, then please indicate ALL necessary phone numbers for us to contact your insurance company and all correct I.D. numbers listed on the card).
3. When all forms are completed, please bring them to our office at your earliest convenience so we may schedule your procedure. You may also mail it to our office: 488 E. Santa Clara St., Suite 102, Arcadia, CA 91006.
